As a Federally Qualified Health Center, PCHS’s mission is to provide quality medical, dental, and behavioral health services regardless of a person’s ability to pay. PCHS provides a sliding fee discount program for services based on income, number of family members, and federal poverty guidelines. Depending on qualifications, patients may be eligible for a 100%, 75%, 50%, or 25% discount on services. At new patient registration, patients have the option to apply for the sliding scale fee. Patients will be asked to fill out a form, as well as provide proof of income for all members of the household, proof of address, and a picture ID. Other necessary documents may include: current regular payroll stub, income tax return, and/or utility bill. Call Family Health Services of Alpine for more information and to book appointments.
